As a Home Health Physical Therapist, you’ll help patients regain their strength, manage their pain, and reach their goals in a home setting where therapy is customized to their daily lives. You will be the person who shows up and helps turn things around for individuals struggling with strength and mobility challenges.

You will work as part of a home health team—including RNs, LPNs, CNAs, OTs, PTAs, and SLPs—focused on the patient’s plan of care and goals. Your day will involve observing movements, conducting assessments, documenting progress, and modifying care plans as needed. You will assist patients with exercises and hands-on therapy while educating them and their families on their plan of care, therapy goals, home exercises, and self-care.
