About the Role

We are seeking a dedicated Physical Therapist to provide high-quality home care services to adult and geriatric patients throughout the Bronx. This role is designed for a self-directed professional who thrives in a mobile environment, managing a caseload across diverse home settings while maintaining the highest standards of clinical excellence.

In this position, you will serve as a vital link in the patient's care journey, working closely with nurses, social workers, physicians, and caregivers to ensure seamless, patient-centered care transitions. Your day will involve conducting comprehensive assessments, developing individualized plans of care, and setting measurable functional goals that reflect each patient's unique medical condition, home environment, and personal priorities. You will deliver therapeutic interventions including gait and balance training, transfer training, and pain management strategies, while providing essential education to patients and their families.

Beyond clinical duties, you will identify safety risks within the home, recommend adaptive equipment, and model VNS Health's culture of compassion, integrity, and collaboration.
