In this position, you will serve as a vital member of the care team, evaluating and treating patients with a variety of conditions. Your primary mission is to develop and implement individualized, evidence-based treatment plans designed to enhance mobility, strength, balance, and overall functional independence. You will work closely with physicians, nurses, occupational and speech therapists, and case managers to coordinate comprehensive, patient-centered care.
A significant part of your day involves educating patients and their families on diagnosis, prognosis, safety strategies, and individualized home exercise programs to promote carryover and prevent complications. You will monitor patient responses to interventions, reassess regularly, and modify treatment plans based on clinical outcomes and patient goals. By adhering to all system policies regarding infection prevention, safety, and ethical practice, you will ensure the highest standards of care while engaging in ongoing professional development to maintain and enhance your clinical skills.

Experience or continuing education in orthopedic, neurologic, or cardiopulmonary rehabilitation. Membership in APTA or relevant professional associations.
