Our team operates within a collaborative, clinician-led culture that values quality care, professional growth, and teamwork. Our in-house therapy program fosters strong interdisciplinary relationships, ensures consistent clinical standards, and empowers clinicians to truly influence resident outcomes without the pressures of contract turnover.

As a Physical Therapist, you will evaluate, plan, organize, and administer physical therapy services in strict accordance with federal, state, and local standards. Your primary mission is to ensure the highest degree of quality care is rendered at all times. You will maintain informative, accurate, and timely documentation while working collaboratively with all disciplines to adhere to professional standards of practice guidelines and the code of ethics set forth by the state board of physical therapy.
In this role, you will support Occupational and Physical Therapist Assistants by providing periodic training and guidance to enhance patient care. You will develop realistic, documented treatment goals that comply with the intent of referring medical staff and prepare written treatment programs based on patient evaluations. Your day-to-day responsibilities include administering manual exercises, soft tissue mobilization, and instructing patients in various physical activities, such as ambulatory functional activities and the use of assistive devices like crutches, canes, and prostheses. You will also evaluate, fit, and adjust prosthetic and orthotic devices, coordinate with physicians for additional patient information, and instruct patients and families on treatment procedures to be continued at home.
Our in-house therapy staff model is built on a foundation of shared values: Integrity, Respect, Commitment to Excellence, Accountability, and Teamwork. This approach fosters a culture of excellence and collaboration between departments, focusing on individualized care for our residents. By delivering consistent, personalized care through dedicated in-house teams, we ensure resident needs are met through tailored programs and a collaborative Rehab Care Model.
We are currently offering a $5,000 sign-on bonus and relocation assistance for qualified candidates ready to make an impact and call Grants Pass home.
We are committed to equal employment opportunity regardless of race, color, ancestry, religion, sex, national origin, sexual orientation, age, citizenship, marital status, disability, gender, gender identity or expression, or veteran status. We are proud to be an equal opportunity workplace.
