About the Role
Physical Therapists at VNS Health help New Yorkers get back on their feet and out of the hospital so they can heal where they are most comfortable – in their homes and communities. You will be part of our 130-year history and the innovative Future of Care built by clinicians like you.
In this role, you will practice independently in the community as part of an interdisciplinary care team. You will design and deliver personalized 1:1 rehabilitative therapy programs for patients recovering from surgery, illness, or accident. Your day-to-day involves conducting comprehensive assessments to determine patient needs, developing treatment plans and outcomes, and delivering therapy in patients' homes or care facilities. You will constantly evaluate evolving patient needs and respond with plan of care adjustments, assessing progress by being a part of their daily life.
