About the Role
In this position, you will serve as a vital link in the continuum of care, providing therapeutic treatments under the direction of a licensed Physical Therapist. Your primary mission is to help patients regain mobility and independence within the comfort of their own homes. You will implement established care plans, monitor patient progress, and educate both patients and caregivers on home exercise programs, safe mobility, fall prevention, and the proper use of assistive devices.
Your day will involve coordinating daily home visit schedules across the assigned Bronx territory, ensuring efficient time management and route planning. You will document clinical information using electronic health records and maintain open communication with the supervising Physical Therapist and the interdisciplinary care team regarding patient status, concerns, and goal updates. This role requires a strong understanding of rehabilitation principles and the ability to navigate Medicare and insurance requirements while maintaining strict compliance with clinical, regulatory, and safety standards.
