About the Role
In this position, you will provide therapeutic treatments under the direction of a licensed Physical Therapist, helping patients regain mobility and independence in the comfort of their homes. You will implement care plans, monitor progress, and educate patients and caregivers on home exercise programs, safe mobility, and fall prevention. The role involves coordinating scheduling to manage a caseload efficiently while collaborating with nurses, therapists, physicians, and social workers to support comprehensive, interdisciplinary care plans.
