As a Physical Therapist Assistant at H2 Health in Lady Lake, FL, you will become part of a dedicated team focused on delivering personalized, one-on-one outpatient therapy within a luxury senior living setting. This role exists to help patients achieve their rehabilitation goals through close collaboration and tailored care.
You will work closely with patients suffering from a variety of conditions, including orthopedic, neurological, and post-surgical cases. The day-to-day involves implementing individualized treatment plans designed by licensed Physical Therapists, monitoring patient progress, and utilizing electronic medical records for accurate documentation. You will collaborate with a multidisciplinary team to enhance patient outcomes, ensuring comprehensive care in a flexible, full-time schedule with no weekends or holidays.
