We are seeking a compassionate Physical Therapist to deliver skilled home care services to adult and geriatric patients. In this position, you will be the primary driver of patient mobility, function, and quality of life within the comfort of their own homes. You will assess, plan, and execute individualized therapy programs while collaborating closely with an interdisciplinary team of nurses, physicians, and social workers.
Your day will involve conducting comprehensive functional and musculoskeletal assessments, establishing measurable goals with patients and caregivers, and delivering therapeutic interventions such as gait training, balance exercises, and pain management strategies. Beyond clinical treatment, you will play a vital role in patient education, teaching safe mobility techniques, fall prevention, and the proper use of assistive devices. You will also identify home safety issues and recommend necessary environmental modifications to ensure a secure recovery environment.
