About the Role
Encompass Health is seeking a dedicated Pharmacist to join our team in delivering high-quality, integrated healthcare focusing on rehabilitation and home health/hospice services. Our pharmacists play a pivotal role in our commitment to help patients regain independence after serious illnesses or injuries.
In this position, you will manage medication therapy and oversee the medication management system. Your day-to-day involves assessing the appropriateness and effectiveness of patient medications, monitoring health progress to identify potential interactions and side effects, and advising the healthcare team on drug dosage, storage, and safety. You will also assist in creating medication therapy programs and educating patients on their prescribed treatments, ensuring they understand how to take their medications and manage potential side effects. Success in this role requires staying abreast of the latest drug developments and regulations while maintaining accurate records of inventory and prescriptions.
