About the Role
Encompass Health is seeking a dedicated Pharmacist to join our team in delivering high-quality, integrated healthcare focusing on rehabilitation and home health/hospice services. Our pharmacists play a pivotal role in our commitment to help patients regain independence after serious illnesses or injuries.
In this position, you will manage medication therapy and oversee the medication management system. Your day-to-day work involves assessing the appropriateness and effectiveness of patient medications, monitoring health progress to identify potential interactions and side effects, and advising the healthcare team on drug dosage, storage, and safety. You will also assist in creating medication therapy programs and educating patients on their prescribed medications, ensuring they understand how to take them and what to expect.
This role demands a thorough understanding of pharmaceutical care practices and a deep commitment to patient safety. You will stay abreast of the latest drug developments and regulations while maintaining accurate records of medication inventory and patient prescriptions. Success in this position requires the ability to work collaboratively within an interdisciplinary healthcare team and communicate effectively with both healthcare professionals and patients.
