About the Role
Encompass Health is seeking a dedicated Pharmacist to join our dynamic healthcare team, where we are committed to providing top-notch pharmaceutical care in a setting focused on rehabilitation and home health/hospice services. The ideal candidate will be deeply committed to treating the complex medication needs of patients recovering from serious injuries or illnesses, with a sharp focus on patient education and safety.
This role is pivotal in ensuring that all patients receive the highest standard of pharmaceutical care tailored to their specific rehabilitation journeys. You will collaborate closely with healthcare providers to optimize pharmaceutical care plans, reviewing and managing patient medication therapy while providing essential counseling. Your expertise will be instrumental in offering drug information and recommendations to doctors and nurses, while monitoring drug therapy regimens for contraindications, interactions, allergies, and dose appropriateness.
In addition to direct patient care, you will supervise and verify the accuracy of all work performed by pharmacy technicians and interns, ensuring a seamless and safe environment for our patients.
