The Patient Services Coordinator (PSC) serves as a vital hub for scheduling patient services and coordinating closely with our staff. This role is designed to ensure seamless operations within our agency while fostering strong relationships across all segments of the team. In addition to core scheduling duties, the PSC is expected to cross-train as a Medical Records Specialist (MRS) to support our broader operational needs.
This position operates primarily within a routine office environment during agency operating hours. The role may involve a moderately high noise level and requires the ability to work a flexible schedule, including extended hours. Depending on clinical requirements, the role may involve local travel and exposure to variable weather conditions. Staff may also encounter standard office elements such as toner or other materials.
Candidates must be high school graduates; graduation from an accredited school of vocational nursing is preferred. For clinical aspects of the role, a current state license as a Licensed Vocational Nurse (LVN) or Licensed Practical Nurse (LPN) is required. A multi-state license issued by a state member of the National Nurse Licensure Compact (NLC) is preferred. A valid driver's license and reliable transportation with current auto liability insurance are mandatory. If patient visits are required, a current CPR card is necessary.
One year of experience in homecare, such as home health or hospice, is preferred. The ideal candidate possesses strong interpersonal skills and the ability to establish effective working relationships. Clinical staff must demonstrate nursing skills aligned with generally accepted standards of practice.
The PSC is responsible for completing workflow tasks within the agency's electronic medical record and prioritizing the scheduling of new patient admissions in coordination with Administration or Case Management RNs. The role involves active participation in care coordination with nurses, patients, and interdisciplinary team members. Effective communication with patients, staff, healthcare professionals, and referral sources is essential.
The PSC must respond to agency and patient needs professionally and creatively, demonstrating a commitment to professional growth and competency. Participation in agency-sponsored in-service training and the Quality Assurance and Performance Improvement (QAPI) program is required. Clinical staff will perform assigned patient visits and ensure timely documentation. The role also involves promoting the agency's philosophy and administrative policies to ensure high-quality care.
