About the Role
This position is a vital part of our integrated primary care team, working remotely from South Dakota. The role exists to ensure the success of patient care through proactive prevention and wellness strategies. You will support a specific clinic by managing and coordinating care for a panel of patients, acting as a bridge between patients, providers, and resources.
In this role, you will monitor patient attribution to ensure the integrity of our electronic health records, identify gaps in care, and coordinate preventative services. Your day-to-day involves making outreach to patients and healthcare providers to ensure seamless coordination, connecting patients to appropriate services to promote optimal health and wellbeing. You will assist the care team in managing preventative screenings, supporting office visits and procedures, and managing chronic disease registries and provider scorecards.
You will also participate in the care of patients across all ages, collecting subjective and objective data to identify potential barriers to care, such as social determinants of health (SDoH), and referring them to care management roles as appropriate. Working within a pre-defined scope of practice, you will track and report quality measures, validate the accuracy of patient records, and communicate collected data to support the healthcare team's plan of care.

Requirements
