About the Role
We are seeking a Family Nurse Practitioner for our East New York Health Center, located at 999 Blake Avenue, Brooklyn, NY. Reporting to the Medical Director, you will be an integral part of a collaborative care team, providing evidence-based Primary Care to our patients.
In this role, you will work alongside nursing staff and other essential services, including Health Education, Nutrition, Social Work, and Behavioral Health, to deliver comprehensive care to medically underserved communities throughout New York City.
