Key Responsibilities
- check_circlePerform Annual Wellness Visits and physical examinations for members
- check_circleDeliver preventive health education and counseling to patients
- check_circleIdentify clinically relevant diagnoses for risk adjustment documentation
- check_circleClose quality care gaps including screenings and follow-ups
- check_circleMaintain accurate and compliant EMR documentation
- check_circleCollect vital signs and perform medical history reviews
- check_circleTravel to members' homes to conduct comprehensive health evaluations
- check_circleGenerate post-visit summaries for primary care providers
- check_circleConduct preventive health screenings such as HbA1c and spirometry
- check_circleReview and reconcile patient medications
Requirements
- verifiedValid unencumbered NP License for the state of work
- verified3 years patient care experience preferred (primary care/adult/geriatric, EMR)
- verifiedPrevious in-home risk assessment experience preferred
- verifiedPrevious 1099 (PRN) experience preferred
- verifiedAccess to reliable transportation
- verifiedStrong ability to work within EMR system
- verifiedAbility to work independently
- verifiedBilingual is a plus
