The Nurse Navigator is a leader providing support, mentorship, and guidance to those who provide direct patient care. Based at Mercy Hospital, St. Louis - Vascular Specialists, this position is designed to facilitate an effective process of the new care model, supporting quality patient care, safety, and financial components.
You will coordinate the delivery of patient care by assessing individual requirements and making recommendations based on severity of illness, intensity of service, and skill level. Your day-to-day involves ensuring patients receive timely, safe, effective, and high-quality care by coordinating the plan of care with the bedside nurse. You will set expectations regarding professional conduct and serve as a resource for the patient care unit, holding direct and indirect care providers accountable for their professional responsibilities and adherence to regulatory guidelines.
This role manages an assigned inpatient and outpatient population, promoting the integration of a seamless care model. Key activities include coordinating patient discharge planning for post-acute services, managing length of stay (LOS), optimizing performance, and adopting best practices. You will also lead pre-op education, perform daily rounding while the patient is in the hospital, determine patient discharge disposition, and manage follow-up care.
