We are currently seeking a Nephrology Nurse Practitioner to join a multi-specialty practice in the Bronx, New York. This position offers a unique opportunity to provide specialized care to patients with kidney-related conditions within a collaborative and supportive outpatient setting.
In this role, you will serve as a vital link in the continuum of care for individuals managing chronic kidney disease, hypertension, and other renal conditions. The day-to-day involves partnering with supervising nephrologists to develop individualized treatment plans, educating patients and families on disease management and dietary modifications, and coordinating care with dialysis centers and transplant teams. You will work in a mission-driven environment dedicated to improving patient outcomes, with the flexibility to choose between full-time or part-time schedules (minimum of 3 days for part-time).
