About the Role
As a Remote MTM Pharmacist at Signify Health (a subsidiary of CVS), you will be a vital member of the MTM product team. Your primary focus will be delivering high-quality medication therapy management services, specifically conducting outbound calls for comprehensive medication reviews (CMR).
In this role, you will provide in-depth consultations regarding statin medication use in strict adherence to AHA guidelines. A key part of your day involves transcribing patient-specific prescription information, diagnosis details, and clinical recommendations into a third-party, web-based application. Whether the source data is hard-copy or digital, your accurate entry will be used to generate standardized documents that are mailed directly to individual patients.

Requirements
