As a Mental Health Associate at Grace Medical Center, you will provide essential psychosocial rehabilitative services to individuals with severe and persistent mental illness within our Partial Day Program. This role is designed to support adults and children in regaining independence and improving their quality of life.
In this position, you will teach and assist members with Activities of Daily Living (ADLs), such as shopping, budgeting, food selection, safety skills, and mobility training. You will conduct one-on-one and group sessions focused on skill development and facilitate therapeutic activities alongside the PHP-A staff. Your work extends beyond the facility; you will perform outreach to clients in the community as indicated on their Individualized Recovery Plans (IRP) and assist with care coordination duties, including linking patients to community resources and scheduling medical follow-ups.
You will be an integral part of a team environment, participating in staff education, policy improvement initiatives, and quality improvement activities. This role offers a unique opportunity to work at the top of your license within a system that values innovation and outcomes.
