The Member Care Services Associate for Quest Integration (QI) serves as a vital advocate and navigator for Kaiser Permanente members and their families, specifically those with special health care needs. This role exists to bridge the gap between complex medical, social, financial, and personal challenges by providing comprehensive case management services. You will work directly with the QI RN Manager to ensure that adults, seniors, and children are matched with the appropriate care managers and coordinators, fostering a seamless integration of care.
In this position, you will act as the primary liaison between members, families, and the broader Kaiser Permanente system. Your day involves interpreting QI program services, confirming coverage, and guiding members through the healthcare landscape. Whether navigating transportation and housing needs for medical services across islands or on the mainland, or addressing social and behavioral difficulties, you will ensure that every member receives the support they require to thrive.
Beyond direct member interaction, you will play a key role in shaping QI processes. This includes coordinating monthly collaborative Care Rounds, developing tracking systems for health functional assessments, and creating care plan summary templates for the Kaiser EHR. You will also build and maintain a "toolbox" of resources for initial and reassessment home visits, ensuring that care managers and coordinators have the necessary materials to deliver high-quality, consistent care.
