We are seeking an experienced Certified Medical Assistant to join our team as a Transitional Care Specialist (TCS). This full-time role is designed to support patients as they transition from inpatient, emergency department, skilled nursing, or acute rehab settings back to the primary care environment.
As a TCS, you will enhance patient outcomes by coordinating follow-up care and conducting structured post-discharge outreach. Your day-to-day involves ensuring timely communication between care teams and addressing clinical or social needs that may impact recovery. You will work closely with providers, nursing staff, care managers, and administrative teams to maintain continuity of care and help reduce avoidable readmissions.
