Voorhees Pediatric Facility is a nationally recognized pediatric specialty care center dedicated to advancing the full life potential of medically fragile children, adolescents, and young adults. Our mission is to provide excellent holistic care in a supportive environment. As the home of the largest freestanding pediatric ventilator program in the country, we offer 119 in-patient beds and have expanded our services to include The Bancroft School, Medical Daycare, and Rehabilitation Services. In 2013, we received the highest accreditation awarded by The Joint Commission, cementing our status as a leader in pediatric healthcare.

Key Responsibilities
- check_circleComplete and submit MDS assessments accurately within federal and state timelines
- check_circleCoordinate and lead the Resident Assessment Instrument (RAI) process
- check_circleInitiate and complete Care Area Assessments (CAA) and Significant Change in Status Assessments (SCSA)
- check_circleDevelop and revise individualized care plans based on MDS findings
- check_circleAnalyze MDS data to identify opportunities for improvement and support PDPM reimbursement
- check_circleMonitor the Case-Mix Index (CMI) for NJ Medicaid and ensure regulatory compliance
- check_circleServe as the primary liaison, educator, and supervisor for MDS-related care
- check_circlePrepare for and participate in NJDOH surveys, audits, and quality reviews
Requirements
- verifiedCurrent New Jersey Registered Nurse license in good standing
- verified1–2 years of experience in long-term care or subacute rehabilitation
- verifiedProficiency in electronic health records and MDS software systems
- verifiedStrong knowledge of RAI Manual, PDPM, and NJDOH Long Term Care regulations
- verifiedCertification as a Resident Assessment Coordinator (RAC‑CT) preferred
