About the Role
The LVN Care Coordinator serves as a vital link in our home care delivery system, ensuring that patient care is coordinated, managed, and delivered with the highest standards of quality. This role exists to bridge the gap between clinical execution and strategic oversight, supervising clinical personnel while guiding the development of comprehensive care plans.
In this position, you will manage the full lifecycle of patient cases, from receiving referrals and assigning appropriate clinicians to conducting quarterly record reviews. Your day involves evaluating clinical diagnoses, medications, and procedures, while actively assisting clinicians in setting therapeutic goals and priorities. You will be the go-to resource for clinical staff during operating hours, offering guidance to promote effective performance and quality service delivery.
Beyond direct patient care coordination, you will play a key role in organizational growth by participating in the screening and interviewing of new personnel, assisting with orientation, and supporting the development of inservice education programs. You will also contribute to the formulation of policies and procedures and represent the organization in community activities to promote our role in the healthcare delivery system.
