About the Role
Encompass Health seeks an LPN/LVN to deliver compassionate, high-quality nursing care in our inpatient rehabilitation hospital. In this role, you will support patient-centered recovery by administering medications and treatments, monitoring progress, and assisting with mobility and ADLs. You will collaborate with an interdisciplinary team to execute care plans, educate patients and families, and ensure a safe, nurturing environment that promotes optimal rehabilitation outcomes and offers opportunities for professional growth.

Key Responsibilities
- check_circleDeliver direct nursing care to rehabilitation patients under RN and physician direction
- check_circleAssist with patient assessments, care planning, and interdisciplinary team conferences
- check_circleDocument care accurately in the EHR to ensure regulatory and facility compliance
- check_circleSupport patient mobility, ADLs, and therapy goals to promote optimal recovery
- check_circleEducate patients and families on rehabilitation processes, medications, and self-care
- check_circleMaintain infection control, safety standards, and a clean therapeutic environment
- check_circleCollaborate with therapists, RNs, and physicians to coordinate patient-centered care

Requirements
- verifiedCurrent LPN/LVN license in state of practice
- verifiedBLS (Basic Life Support) certification
