This position promotes a healthy lifestyle by managing the resident care experience through effective communication flow between the community's onsite Medical Clinic, therapies, ancillary services, and the interdisciplinary team. You will serve as a vital liaison between families, residents, and healthcare professionals, fostering a continuum of care.
In this role, you will develop partnerships with service providers and agencies for resident referrals and educate residents on available services and monitoring provisions. Your day-to-day involves conducting reviews to determine the feasibility of continued residence and health care needs, while ensuring the delivery of services is appropriate, timely, and satisfactory. You will also assist management in identifying residents who need assistance and coordinate that support, responding to emergency requests as part of the response team.
