About the Role
We are seeking a compassionate and detail‑oriented Licensed Practical Nurse (LPN) to serve as our Case Manager within our Skilled Nursing Team. Reporting to the Health Services Administrator, this role is essential to ensuring a smooth, welcoming, and clinically sound transition for residents entering our community.
As the Admissions Nurse/Case Manager, you will act as the primary clinical liaison between hospitals, physicians, families, and internal departments. Your day will involve coordinating new admissions and readmissions, reviewing clinical documentation for compliance, and completing initial nursing assessments. You will collaborate closely with therapy, pharmacy, dietary, and social services teams to support seamless transitions of care. Beyond clinical duties, you will educate residents and families about facility services, ensuring a positive onboarding experience while promoting resident safety and infection control standards.

Requirements
