About the Role
We are seeking a dependable Licensed Practical Nurse (LPN) to join our Continuing Care team in Grand Rapids, MI. This position is designed for a professional who thrives in a night shift environment and takes pride in accurate documentation, patient safety, and seamless teamwork.
In this role, you will provide direct patient care under the direction of an RN, mid-level provider, or physician. You will collaborate closely with the interdisciplinary care team to ensure safe, respectful, and high-quality care for our residents. The position offers consistent hours with a 12-hour night shift schedule (7:00 PM–7:30 AM) and every other weekend, with no on-call requirements.
Team Context
Our Continuing Care setting focuses on long-term support, requiring a nurse who is comfortable with EMR/EHR charting, fast-paced workflows, and effective team communication. You will play a vital part in maintaining a clean patient environment, performing clinical documentation, and participating in quality monitoring and safety initiatives.
