About the Role
We are seeking a Licensed Practical Nurse to deliver compassionate home care services to patients in the Bronx. This role is vital for ensuring skilled nursing care reaches individuals in their own homes, allowing them to maintain independence while receiving professional medical support. You will serve as a key member of an interdisciplinary team, working under the supervision of RNs and physicians to implement and monitor individualized care plans.
In this position, you will be the face of VNS Health during home visits, building trust with patients and their families. Your day will involve assessing patient conditions, administering treatments, and providing essential education on disease management and self-care. Beyond clinical duties, you will coordinate visit schedules and manage routes throughout the Bronx, ensuring efficient and timely care delivery.
