The Integrated Health Therapist (IHT) serves as a vital resource for patients and team members, addressing mental and behavioral health, chemical health, and the psychosocial aspects of disease. This role is designed to support optimal patient functioning through a bio-psychosocial conceptualization of health. You will provide diagnostic assessments, onsite crisis intervention, and empirically validated short-term behavioral health services to patients of all ages, including those with co-morbid medical conditions.
In this position, you will work collaboratively with the PCMH or PCC team to address the cultural, developmental, and economic factors influencing a patient's ability to manage acute and chronic conditions. You will act as the primary team consultant, promoting understanding of the relationship between health and psychological factors among patients, families, and the community. Your day-to-day responsibilities include leading universal health screenings for behavioral and chemical health conditions, providing education on behavioral emergency management, and ensuring quality delivery of care through innovative models like telehealth.
Credentialing through the Allied Health Staff of the organization is required for this position. Candidates must hold a Master's degree in Counseling, Social Work, Psychology, or a related Behavioral Health Sciences field from an accredited school. A valid license in good standing in the state of practice is mandatory. In some instances, candidates seeking licensure by reciprocity may be considered if they can obtain licensure within 90 days of hire, provided they can demonstrate the ability to meet state statutes and requirements.
