About the Role
This Full-time Clinician (Nurse Practitioner or Physician Assistant) role is designed for those with a passion for engaging with people and helping them on their journey to better health. You will provide In-Home Health Evaluations, traveling up to 100% of the time across a defined coverage area in New Hampshire. This position requires flexibility to travel locally and within licensed states, which may include overnight hotel stays, car rentals, and/or flights depending on business needs.
In this role, you will play a critical part in building trusted relationships to make people healthier. The typical visit lasts about 45 minutes, allowing time to connect one-on-one and answer health questions. You will conduct a medication and medical history review, perform a physical evaluation, and, if ordered by the person's health plan, complete additional diagnostic tests such as diabetic eye exams or spirometry. You will utilize an iPad and an intuitive clinical workflow for each evaluation. Notably, you are not required to prescribe medicine, order lab tests, or alter people's current treatment regimen, allowing you to focus entirely on the clinical connection and education.
As an in-home clinician, your primary workplace is the private residence of health plan members. These environments are diverse and may present a variety of conditions, including household pets, secondhand smoke, and other potential airborne allergens or irritants. Candidates must be able to comfortably and safely perform their duties in these settings.
