As a Hospice Nurse, you will render professional nursing services to patients and families, demonstrating the ability to assess, plan, evaluate, and implement nursing care with attention to the special needs of the agency. You will work in accordance with the Team's Plan of Care and written agency programs and policies, serving as a primary resource for comprehensive nursing care, health teaching, guidance, and supervision.
Your day will involve collaborating with an interdisciplinary team to assure appropriate care planning and communicating with the patient's physician and/or hospice Medical Director. You will assume responsibility for keeping informed of current changes and trends affecting hospice care and professional nursing functions. A significant part of your role includes participating in the bereavement program, supervising volunteers in providing bereavement care, and considering the psychosocial, environmental, spiritual, and financial needs of the patient and family.
You will also supervise patient care given by LPNs and/or HHAs, maintaining timely, accurate, and clear clinical records that outline goals and plans with an emphasis on a family-oriented approach. By keeping administration informed on changing community needs and participating in the orientation of new staff, you will contribute to the development and operation of the agency while modeling the Network's Service Excellence Standards of Performance.
