As a Clinical Manager at CenterWell Home Health, you will lead and support a team of dedicated clinicians who deliver compassionate, high‑quality care in the home setting. In this position, you will act as a bridge between clinical excellence and operational efficiency. You will review referrals, determine admission appropriateness, and assign clinicians to ensure Plans of Care meet patient needs. Your day will involve guiding, supporting, and educating clinicians on goal-setting and clinical decision-making, while remaining available during operating hours for direct support.
You will also coordinate communication among physicians, team members, and caregivers to support care coordination and discharge planning. In exceptional or unplanned circumstances, you may provide direct patient care and act as the Branch Director in their absence. This role requires a balance of strategic oversight and hands-on leadership to foster a culture of continuous improvement and quality outcomes.
This role is considered patient-facing and is part of Humana's Tuberculosis (TB) screening program. If selected, you will be required to be screened for TB. Additionally, as this role is part of Humana's driver safety program, you must maintain a valid state driver's license and personal vehicle liability insurance in accordance with state minimum required limits.
