About The Arc San Francisco
The Arc San Francisco is a non-profit service and advocacy organization dedicated to adults with autism, Down syndrome, cerebral palsy, intellectual and developmental disabilities, and their families across San Francisco, Marin, and San Mateo counties. We currently serve over 550 participants who share a common desire to live lives of greater self-determination, dignity, and quality.
Our mission is to provide the supports necessary for people with developmental disabilities to maximize their potential, live meaningful lives, and become integral members of the community. We offer a wide range of services covering all areas of adult life, from independent living supports and skills building to employment training and creative expression. Key Responsibilities
- check_circleCoordinate care across multiple providers and community agencies to improve health outcomes
- check_circleAdvocate for appropriate care and services for participants within the health care system
- check_circleCoordinate integration of complex services and special needs for participants
- check_circleManage scheduling and transportation for medical and other appointments
- check_circleCommunicate timely updates following medical appointments or changes in participant status
- check_circleMaintain current Salesforce files and complete regular documentation and reports
- check_circleManage caseloads of individuals with varying degrees of complex health needs
- check_circleSupport individuals in navigating the health care system and adhering to care plans
- check_circleMake referrals to appropriate medical, behavioral, social, and community resources
- check_circleProvide education and promote self-management to empower individuals in their health
Requirements
- verifiedBachelor's degree in social work or public health preferred
- verifiedOne year experience working with individuals with developmental disabilities preferred
- verifiedExperience working in health care settings preferred
- verifiedValid California Driver's license
- verifiedSafe driving record
- verifiedAcceptance as a covered driver by The Arc's auto insurance company
Nice to Have
Advanced degree and/or specialized training or certification in aging or case management. One year or equivalent experience working with individuals with developmental disabilities. Experience working in health care settings.
