About the Role
As a Direct Care Worker, you will be the compassionate heart of our patient-centered care model. This role exists to support patients with daily living activities, helping them maintain their independence and dignity in both home and facility settings. You will provide hands-on personal care, assisting with mobility, hygiene, and meals, while serving as the eyes and ears of the clinical team by observing and reporting changes in patient condition.
Your day will involve working closely with nurses and other caregivers to implement individualized care plans. Whether you are assisting with bathing, grooming, or safe feeding, your goal is to promote a safe, clean, and dignified environment. You will document care accurately and uphold our core values of compassion, teamwork, and patient-centered care in every interaction.

Requirements
- verifiedCurrent CPR/First Aid certification preferred
- verifiedCertified Nursing Assistant (CNA) or Home Health Aide (HHA) credential preferred
- verifiedValid driver's license and reliable transportation if providing in-home care
- verifiedAbility to pass background check and health screenings
- verifiedKnowledge of basic infection control and HIPAA standards
Nice to Have
