We provide a full range of outpatient services for clients with orthopedic problems, work-related injuries, sport injuries, and neuromuscular and neurological conditions.
This hybrid position invites you to ignite your career as a Physical Therapist and Digital Health Specialist. You will split your time between our CORA Oakleaf clinic in Jacksonville, FL, and remote work, dedicating three days in-person and two days to digital health services.\n As a Digital Health Specialist, you will serve as a vital liaison between patients and in-clinic treating providers. Your day-to-day involves developing and delivering personalized plans of care for a diverse patient population, utilizing cutting-edge software to objectively measure outcomes. You will leverage the digital platform to deliver telehealth visits, including evaluations, assessments, and treatment plan development, while performing Remote Therapeutic Monitoring (RTM) services. Beyond direct patient care, you will actively participate in the development of the digital health program, supporting operations to meet patient care needs and collaborating with the Director of Digital Health to optimize and grow remote services.
Candidates must be located in Jacksonville, FL (Oakleaf) or within driving distance to this hybrid role. The position is full-time, requiring 40 hours per week.
