This hybrid position is designed for a Physical Therapist to serve as a Digital Health Specialist, bridging the gap between in-clinic care and remote patient support. You will work 3 days in person at our CORA Riverside clinic in Jacksonville, FL, and 2 days remotely. In this capacity, you will provide digital health services, including virtual care and remote therapeutic monitoring, while participating in the development of our digital health program.
As a liaison between patients and in-clinic treating providers, you will develop and deliver personalized plans of care for a diverse patient population with both orthopedic and neurologic diagnoses. Your day-to-day involves objectively measuring patient outcomes using cutting-edge software, efficiently documenting evaluations and treatments, and utilizing our digital platform to deliver telehealth visits. You will also perform Remote Therapeutic Monitoring (RTM) services, maintaining regular reporting and data analysis of regional service metrics. Additionally, you will support operations to meet patient care needs and collaborate with the Director of Digital Health to optimize and grow remote services.
Candidates must be located in Jacksonville, FL (Riverside) or within driving distance of the clinic. This is a full-time role (40 hours/week) requiring a dedication to providing exceptional quality of care. While a Florida Physical Therapy state license is required, you must also be willing to obtain licensure in other states as needed.
