About the Role

This hybrid position invites you to ignite your career as a Digital Health Specialist within our Jacksonville, FL (Oakleaf) clinic. You will split your time working three days in person at the clinic and two days remotely, dedicating 40 hours per week to this full-time role.

As a Digital Health Specialist, you will serve as a vital liaison between patients and in-clinic treating providers. Your day-to-day involves developing and delivering personalized plans of care for a diverse patient population with both orthopedic and neurologic diagnoses. You will utilize cutting-edge software to objectively measure patient outcomes and efficiently document evaluations, treatments, re-evaluations, and discharge notes.

A core part of your mission is expanding our digital health program. You will use our digital platform to deliver telehealth visits, including evaluations, assessments, and treatment plan development. Additionally, you will perform Telehealth and Remote Therapeutic Monitoring (RTM) services, engaging in interactive communication and monitoring to support patient care. You will work closely with operations to meet staffing needs, communicate treatment revisions to primary clinicians, and maintain regular reporting on regional service metrics. You will also participate in program development and marketing efforts directed by the Director of Digital Health.

Hiring Process

Candidates must be located in Jacksonville, FL (Oakleaf) or within driving distance of the clinic. The role requires a diploma from a CAPTE-approved Physical Therapy Program and a Florida Physical Therapy state license (or in process). While a minimum of two years of relevant outpatient clinical experience is required, experience providing digital health services is preferred.
