As a Day Habilitation Provider, you will work as a collaborative team member to assist participants with developmental disabilities in their recreational activities and daily living skills. Your focus will be on implementing participant plans of care (POCs), promoting each participant's freedom of choice and independence, and ensuring activities are provided appropriately according to their unique needs.
Your day will involve assisting with basic personal needs, including meal preparation and personal care, while maintaining accurate documentation of progress in electronic medical records. You will also transport participants in the community, ensuring safety and compliance with agency procedures. This role requires active participation in developing care plans, communicating with care coordinators, and attending training sessions to maintain professional standards.
