The Clinical Rehabilitation Specialist serves as an Acute Rehabilitation content expert, responsible for the development and implementation of educational and community relations activities to support awareness and utilization of the rehabilitation program's services. This role is vital for driving program development, quality improvement, problem-solving, and productivity enhancement within a flexible team environment.
In this position, you will conduct thorough patient assessments to identify candidates for potential admission and complete detailed Pre-Admission Screens in accordance with facility, KHRS, and payer requirements. You will review patient medical charts to understand pharmaceuticals, test results, therapy evaluations, and pre-existing conditions. A key part of your day involves gathering clinical information and past history while clearly and professionally interacting with patients, families, and healthcare providers.
You will also engage in target area research to analyze like programs and alternative levels of care. The role requires scheduling meetings and arranging conferences for medical professionals, including potential and existing referral sources, doctors, nurses, social workers, and other healthcare professionals. You will provide patient updates to physicians, payers, case managers, and social workers while maintaining solid working relationships with referral sources through excellent service. Additionally, you will participate in direct contact with patients for clinical and educational services as requested and develop educational and community relations plans in consultation with the Program Director, Medical Director, and host hospital CEO.
