About the Role
As a Clinical Nurse, you will be the cornerstone of safe, evidence-based care for our patients, delivering support in both in-center and home dialysis settings. This role exists to ensure every patient receives comprehensive monitoring and personalized attention throughout their dialysis journey. You will collaborate closely with nephrologists, advanced practice providers, dietitians, social workers, and other interdisciplinary team members to coordinate holistic care plans.
In this position, you will go beyond clinical tasks to foster patient education and empowerment. You will guide patients and families through understanding kidney disease, dialysis treatments, medication management, and self-care strategies. Your day will involve managing vascular access sites, administering therapies, and responding to emergent situations with sound clinical judgment. You will also contribute to quality improvement initiatives and audits, helping to drive excellence in renal care outcomes.

- check_circleManage and monitor vascular access sites including fistulas, grafts, and catheters while recognizing complications
- check_circleDevelop, implement, and update individualized care plans in collaboration with the interdisciplinary healthcare team
- check_circleProvide patient and family education on kidney disease, dialysis treatments, medications, diet, and self-management strategies
- check_circleMaintain accurate, timely clinical documentation within the electronic medical record and follow all regulatory standards

Requirements
- verifiedActive Registered Nurse (RN) license in the state of practice
