As a Clinical Liaison, you will serve as a vital bridge between our facility and the broader healthcare community. You will advocate for referred patients during the pre-admission process, collaborating with clinical teams to assess complex patient needs. Your role involves fostering relationships with medical facilities to serve as a trusted resource for healthcare professionals.
You will utilize your clinical expertise and problem-solving skills to coordinate care plans with physicians and families, ensuring a smooth continuum of care after ICU or post-surgical stays. A key part of your day will involve facilitating patient transfers and actively addressing barriers—whether payer-related or operational—that prevent patients from accessing our specialized post-ICU care.
You will be a champion of The Select Medical Way, which centers on putting the patient first, improving the quality of life in your community, and doing what is right by providing high-quality care.
