About the Role
As a Clinical Care Nurse at Conviva Senior Primary Care, you will deliver compassionate, coordinated care to adults aged 65 and older. This role exists to support providers in managing the unique health needs of seniors, with a specific focus on prevention, wellness, and chronic disease management. You will be the bridge between clinical assessments and patient well-being, educating patients and families while helping them navigate community resources.
Working within an interdisciplinary team, you will contribute to a respectful, patient-first culture that values long-term relationships, continuous learning, and improving seniors' health, independence, and quality of life. Your day-to-day involves partnering with physicians and care team members to develop individualized care plans, monitor clinical outcomes, and ensure compliance with safety standards and regulatory requirements.

Key Responsibilities

- check_circleDevelop, implement, and update individualized care plans with the care team
- check_circleEducate patients and families on conditions, medications, and wellness strategies
- check_circleCoordinate care, referrals, and follow-up with physicians and interdisciplinary team members
- check_circleMonitor clinical outcomes, document accurately in the EHR, and report changes in condition
- check_circleParticipate in quality improvement, population health, and preventive care initiatives
- check_circleCollaborate in case conferences and team huddles to support value-based care goals
- check_circleEnsure compliance with clinical protocols, safety standards, and regulatory requirements
Requirements
- verifiedActive RN license in practicing state
- verifiedBLS certification
- verifiedACLS certification preferred
- verifiedExperience in geriatric or primary care settings preferred
