About the Role

As an Inpatient Care Manager at Northwest Hospital, you will collaborate with the clinical team and medical providers to provide discharge planning coordination and intervention. This role is designed to promote patient wellness, improve care outcomes, ensure efficient utilization of health services, and minimize payment denials for a patient population with complex health needs.\n You will join a team that values innovation and outcomes, delivering life-saving care to the youngest and most vulnerable patients.
