We are excited to announce an opportunity for a full-time Case Manager to join a groundbreaking new program dedicated to addressing health-related social needs and improving overall outcomes for Medicaid recipients. This innovative initiative connects individuals to essential community resources that support their overall well-being and health outcomes.
In this role, you will be the bridge between complex health needs and the community resources that can address them. You will conduct comprehensive intake and eligibility assessments, facilitate service navigation, and ensure participants receive timely and effective care coordination. Working within a collaborative team environment, you will utilize a strengths-based approach to support diverse and underserved populations, helping them overcome barriers to access vital services.
