The Care Navigator serves as a vital link within the Care Navigation Center, working closely with the team to coordinate cases on a day-to-day basis. This role is essential for ensuring the smooth disbursement of case-related materials, scheduling care navigation appointments, and tracking utilization. You will be the first point of contact for processing incoming requests, initiating cases in the designated case management system, and ensuring all documentation meets security and HIPAA requirements.
In this position, you will manage the flow of information between service recipients, providers, and consultants. Your day will involve triaging cases, monitoring service inboxes, and making satisfaction calls to ensure high-quality service delivery. You will also play a key role in operational improvements, helping to develop new service lines and ensuring providers adhere to departmental processes.
