About the Role
We are seeking a Care Coordinator to join our Behavior Health Care Coordination (BHCC) program in Jamaica, Queens. In this role, you will provide a variety of health home services to ensure that an individual's physical, mental, and health needs are addressed in a comprehensive manner. You will oversee and facilitate access to the services necessary for clients to stay healthy, avoid emergency room visits, and prevent hospitalizations. Your work will focus on creating recipient-centered, strength-based plans of care, coordinating services across various systems, and promoting evidence-based wellness and prevention strategies tailored to individual needs.
