As a Physician Assistant at Self Regional Healthcare, you will play a vital role in delivering comprehensive care to our patients. You will complete medical histories and physical exams, providing treatment to HMSS patients while offering consultative services to appropriate specialty groups. Your work will be guided by the medical plan of care and critical pathways established by Self Regional Healthcare, Self-Medical Group, and Medical/Nursing Policies, Procedures, Protocols, and Standards of Care.
Your day-to-day responsibilities include managing admission and discharge medication reconciliation, ordering and interpreting lab and imaging studies, and completing discharge summaries. You will work collaboratively with physicians, case management, patients, and families to ensure seamless care coordination. Additionally, you will initiate advance care planning and partner with supportive care and hospice teams to assist patients and families with end-of-life decisions.
