As a Community Health Worker (CHW), you will conduct screenings and individual needs assessments in family homes and other community venues. Your primary focus is engaging clients, implementing screening tools, and identifying areas of need to develop actionable plans. You will ensure appropriate referrals are made to matched services and provide ongoing support and monitoring. This role involves close collaboration with the Department of Children, Youth, and Families (DCYF) to manage cases that may remain open for up to six months.
