About the Role
As a Triage Medical Assistant at Primary Health Solutions, you will serve as the first level of support for patients calling into our organization. Working closely with triage nurses, you will manage the initial intake of patient inquiries, ensuring they receive timely and compassionate care. This role is vital in bridging the gap between patients and providers, handling everything from medication refills and appointment scheduling to clinical support and care coordination.
A Day in the Life
Your day will be a dynamic blend of clinical support and customer care. You will answer calls on a multi-line system, applying active listening and behavior modification principles to discuss goals with patients. You will manage the workflow for medication refills, assist patients with routine questions, and document all communications in patient charts.
Beyond the phone, you will act as a liaison to pharmacies, advise patients on payment options and assistance programs, and facilitate care coordination following hospital or ER visits. You may also engage in population health communications, reaching out to remind patients of preventive or chronic care steps. Success in this role requires the ability to work effectively as part of a team, adapting to the needs of diverse patient populations, including geriatric patients and those from lower socio-economic sectors.
